National RN Population Health Nurse Certificate Program TM

The National RN Population Health Nurse Certificate Program is designed to prepare Registered Nurses for new responsibilities and emerging roles focused on improving the health of populations in primary, ambulatory and community-based settings. Nurses in these roles build on nursing’s holistic framework to integrate concepts of public health, advanced care coordination, and interprofessional collaboration that support population health management, improved health outcomes and better utilization of health care resources. Content in this program bridges health policy and evidence with population health professional nursing role development and emphasizes concepts of people-centered care. 

Summary of Practice Outcomes

Nurses who successfully complete this program will be able to:

  1. Demonstrate essential knowledge needed to perform new and emerging population health nurse roles needed to improve health outcomes in primary, ambulatory, and community-based settings.
  2. Utilize evidence based strategies consistent with national initiatives designed to ensure people centered care delivery, address social determinants of health, and support health equity.
  3. Use new knowledge to implement Population Health Nurse role development into practice.
  4. Participate in the National RN Population Health Nurse Active Learning Collaborative, designed to support graduates in the application of population health principles into practice.

About the Population Health Nurse Active Learning Collaborative

This is a faculty guided learning experience designed for RNs who have completed the National RN Population Health Nurse Certificate Program. In this learning activity, National RN faculty provide coaching and support as nurses learn to apply principles of population health in practice including accessing data, interpreting trends and designing intervention strategies to improve health outcomes of selected populations. 

Who Should Enroll

  • Registered Nurses hired into Population Health positions or charged with Population Health responsibilities 
  • Registered Nurses responsible for chronic care and/or disease management across populations
  • Registered Nurses new to population health or chronic care management roles
  • Registered Nurses who are responsible for supervision, program development or leadership of nursing Population Health roles and programs
  • Registered Nurses working in positions to advance populations across patient settings including primary care, community-based outpatient clinics, specialty clinics and services, acute (ED, discharge planning and transitional care), home health 

Continuing Education Approval Statement

The activity has been submitted to the Wisconsin Nurses Association for approval to award contact hours.  The Wisconsin Nurses Association is accredited as an approver of nursing continuing professional development by the American Nurses Credentialing Center’s Commission on Accreditation.

Registrants will be informed as soon as contact hours have been approved for the next period. 

This continuing nursing professional development activity was approved by the Wisconsin Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation (ANCC). WNA CEAP Approval Number: (Pending).

Nursing contact hours must be completed by (Date Pending). No certificates will be issued after this date.

Successful Completion

Completion of the learning activities, quizzes and Course Evaluation demonstrates successful completion of the National RN Population Health Nurse Certificate Program and is required for 20 CE hours and the Certificate of Completion to be awarded.

All materials are posted by topic. Topics are posted sequentially to build on each content area covered.

Each topic contains various learning activities, articles, and resources. When you complete the content in each topic, you will take a short quiz to help you self-assess your understanding of the content.

Following completion of the quiz, the next topic will be available for you to open and begin. Although we do not grade coursework, we do monitor and report progress and completion status. It is expected that as an adult learner you are self-motivated and will thoughtfully complete the course learning activities.

Participants who have completed the Program and downloaded their CE Award and Certificate of Completion will then be able to enroll in the Active Learning Collaborative through the link posted to their personal course page.

Scheduled Class

(Blended Learning)

This online learning option is paired with scheduled faculty discussion. With this learning format, participants complete the 2 Sections of the National RN Population Health Nurse Certificate Program online and may attend virtually facilitated faculty discussion.  

Register for the course, receive email confirmation and log in to the classroom to complete course content following the schedule provided.

COURSE DATES

  • Works well for RNs who desire a structured learning experience accompanied by live faculty interaction to reinforce learning
  • Each week complete 3-4 hours of online content
  • Attend virtual faculty led discussion upon completion of each Program Section

This faculty guided online learning option is designed to be competed independently following the course sequencing and faculty prompts embedded in the recorded lectures.  

With this option, register for the course, receive email confirmation and log in and begin.

With this option, register for each section following the order listed, receive email confirmation and log in and begin.
